WARNING, PLEASE READ CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHTS TO SUE PAWRIFIC STYLEZ MOBILE GROOMING LLC.
In consideration for my pet to being permitted to be a client of Pawrific Stylez Mobile Grooming LLC, I make the following representations and agree to ALL of the following policies, procedures, terms and conditions:
I entrust my pet to Pawrific Stylez MOBILE Grooming llc for the purpose of Grooming my pet i understand and am aware that all due care will be taken with my pet for the safety of the pet, groomer or any employees.
I am aware that if my pets coat is matted, I have the following three (3) options:
Pawrific Stylez Mobile Grooming llc has the right to refuse services for any reason it deems. In the event of a pet that cannot be groomed safety, without the danger to the groomer or pet, Pawrific Stylez Mobile Grooming llc will use muzzles, e-collars, slings, scrapes, ect. Grooming services may be stopped mid groom if necessary for safety.
I agree to allow Pawrific Stylez Mobile Grooming llc the right to use my pet(s) name and any images in any media, marketing, advertisements, illustrations, trade or promotion.
Pawrific Stylez Mobile Grooming LLC strongly recommends that your pets be vaccinated against all infections conditions including: Distemper, Bordetella, Parainfluenza, and Parvovirus. It is possible for a vaccinated pet to become ill with an infection condition despite being vaccinated. This is not due to any circumstance or condition in Pawrific Stylez Mobile Grooming LLC salon and you agree that you will not hold Pawrific Stylez Mobile Grooming LLC liable in the Event your pet becomes ill when an infection condition during or after it’s stay.
Pawrific Stylez Mobile Grooming LLC is not responsible for any Pre-existing medical conditions or the aggravation of these conditions, such as heart disease, arthritis, obesity, infections, or any other medical problem that may be affected by the Grooming process. If we feel the safety of your pet or any Salon associate is in jeopardy, a muzzle may be used or services refused/ discontinued. In case of emergency, the owner designates Pawrific Stylez Mobile Grooming LLC as agent and understand that Pawrific Stylez Mobile Grooming LLC will do whatever is appropriate for the well-being of your pet while in our care. In the event of an injury, if we are unable to contact you, Pawrific Stylez Mobile Grooming LLC at its sole discretion, may engage the services of a veterinarian and the expense shall be paid by you. Pawrific Stylez Mobile Grooming LLC offers any members to be present in our facility during the Grooming process. However, Pawrific Stylez Mobile Grooming LLC is not responsible for any injuries caused. Children under the age of 16 must be accompanied by a legal guardian at all times.
I HAVE READ AND FULLY UNDERSTAND THE TERMS OF THE ‘GROOMING POLICIES AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING THIS CONTRACT AND HAVE SIGNED FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTEE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF LIABILITY TO THE EXTENT PERMITTED BY THE LAW AND AGREE THAT IF ANY REMAINDER OF THE AGREEMENT SHALL REMAIN IN FULL FORCE AND EFFECT, I AGREE THAT IT IS INTENDED THAT ALL TERMS OF THE AGREEMENT CONTROL DESPITE ANY PARTICULAR STATUSOR LAW THAT WOULD OTHERWISE PROTECT ME AND MY PET(S).
